New Brunswick Regulator Mandates Incident Reporting

Error reporting has now become mandatory in New Brunswick.  On June 2nd, 2018, the New Brunswick regulator made a monumental change to the way the territory approaches medication errors within pharmacies through ratification of legislation. Now it is a requirement for all pharmacies to have an error reporting system in place. To be compliant of the new law by January 1, 2019, New Brunswick pharmacies must have a way to process documentation and report known, alleged and suspected medication errors, discrepancies, near misses and show the steps taken to resolve the problem.  …Read more …

UK Pharmacy Order to be enacted this April.

The Pharmacy Order  This April 2018, the Pharmacy (Preparation and Dispensing Errors- Registered Pharmacies Order 2018) has provisionally come into force making changes to the Medicines Act 1968.  Passed by the Houses of Lords and Commons last December, the order provides new defences for inadvertent preparation and dispensing errors made by registered pharmacy professionals (registered pharmacists and registered pharmacy technicians, or those supervised by them) working in registered retail pharmacy businesses.
